    · All information held by Somerset Clinic is done so solely for the purpose of maintaining effective treatment. 

    · Personal information such as your name, email address, mailing address, phone numbers and date of birth.

    · Sensitive Personal Data such as race/ethnicity, physical and mental health, relationships. 

    · Safeguarding and risk information.

    Where the information comes from

    · Information can come from you or others that know you. 

    · Referral forms and information submitted through the website enquiry system. 

    · Telephone conversations, emails and verbal conversations with yourself, the individual referring or other involved professionals. 

    Why we need this information and how we use it

    · To provide our services to you. 

    · To ensure the safety of yourself and others.

    · To improve and enhance the provision of our services. 

    · To respond to your requests and queries 

    · To comply with applicable law(s).

    · To enable us to maintain our own accurate accounts and records. 

    · Non-identifiable data may be used for audit purposes. 

    Who we share information with

    · We may only disclose your information to third parties under the following circumstances:

    o The information is disclosed as permitted by applicable law(s) and/or to comply with applicable law(s).

    o The clinician is significantly concerned for your safety or the safety of someone around you. 

    · We can disclose information to a third party (e.g. your GP) only with your consent. A record of this agreement would be documented through the use of a consent to share information form. 

    · Whenever possible, your clinician will discuss the need to share information prior to disclosing any information.

    · For young people under the age of 16, information will not be shared with parents or guardians without explicit consent if the client is deemed Gillick competent. The exception is if the clinician is significantly concerned about the welfare or safety of the young person, or they have reported information regarding the welfare or safety of another person. 

    Information Security

    · We ensure that we protect your personal information that we hold, its confidentially, integrity and availability. 

    · All files are stored electronically. 

    · We ensure there are systems in place such that information is protected, monitored and recoverable.

    · The confidential information we hold includes your demographic information and clinical notes. Should you supply written documentation from another professional, such as a referral letter, then this will also be held. 

    How long we keep your information for

    · All information is securely disposed of 7 years after discharge from the service. For young people, information will be disposed of 7 years after they turn 18. 

    Therapist Confidentiality

    · It is not permitted for the client to disclose any written material including reports or letters without consent from the clinician. 

    · The client is not permitted to video or audio record the session without consent in writing from their treating clinician.
